Wellness Meal Customer Survey
During my retail foodservice rotation at Kaiser Permanente I developed a Wellness Meal pre-survey for cafeteria customers. I developed questions for the survey in order to gauge customer opinions and purchasing habits. Questions on the survey included, what role the consumer had at Kaiser (i.e. employee, volunteer etc.), how often they visit the cafeteria per week, how often they purchase the wellness meal per week, how they perceived the nutritional quality of the meal, and there was also space for the individual to leave any additional comments. I designed how the survey would be distributed and included an incentive (apples). Finally, I accumulated the survey results into an excel document, analyzed the results and compared the sales data.
